Welcome to Perspectives in Care at Coastal Health

Getting Started

Beginning this month, and before each future visit, you will be asked to answer a series of questions via the Perspectives in Care mobile app or web-based portal.

Participation in this program may be a requirement of your provider, and failure to complete this questionnaire prior to each visit may result in being rescheduled.

The easiest way to get started is to download the Perspectives in Care mobile app here. Click on one of the buttons below to download the app to your phone, then complete your enrollment using the Clinic PIN below.

If you prefer not to download the app, you can access Perspectives in Care using a computer, tablet, or smartphone. On the home page, you will see buttons to “Login” or “Enroll (For First Time Users ONLY)”.

Please be sure to save or store the email address and password you used to create your account, as you will need this information each time you log in.
