Dr. David Levin, D.O.
David Levin, DO is a highly trained physician specializing in interventional pain management and physical medicine & rehabilitation, dedicated to helping patients reduce pain, restore function, and improve overall quality of life.
Dr. Levin completed his Interventional Pain Management Fellowship at the University of Louisville, where he gained advanced training in minimally invasive procedures to treat a wide range of acute and chronic pain conditions. He also completed his Physical Medicine & Rehabilitation residency at the University of Louisville, where he served as Chief Resident, demonstrating strong leadership and a commitment to excellence in patient care.
At our brand new San Pablo clinic, Dr. Levin takes a comprehensive, patient-centered approach to care, focusing on identifying the root cause of pain and developing personalized treatment plans.

Our brand new Spine & Pain clinic is located at 14011 Beach Blvd., Unit #120, Jacksonville, FL 32250. We are opening our doors in October, but we are accepting priority appointment requests starting late July.
Coastal Health accepts Medicare, Medicare Advantage, and most major private commercial health insurance plans, including Aetna, Cigna, Blue Cross Blue Shield, Humana (specialty plans), and United Healthcare. We also accept worker's compensation and auto insurance plans.
